• Details of higher education (course name, institution, and start & end month/year)
• Complete employment/business history for the past 10 years (with proper start and end dates)
• If self-employed, kindly share the business commencement date and business proof (GST Certificate/Business Registration/Business Letter)
• Nature of business/occupation

Travel History

• Details of all international travel undertaken during the past 5 years

Medical & Family Details

• Have you undergone a medical examination by an IRCC-authorized panel physician within the last 12 months?
• Any medical history (illness, hospitalization, or ongoing treatment)
